PROCEDURE TO SETUP DEBIT ORDER

1. Please fill in the debit order request form below with your contact details.
2. You will then receive an authorization letter by email asking you to sign and return it to us by email or fax.
3. We cannot process your debit order monthly donation without a signed copy of the authorization letter.
4. At the end of our financial year (31 March), you will receive a tax certificate by email with your annual contributions. This can be submitted with your tax returns as proof of your donation. The Organ Donor Foundation has Section 18A status. Any donation over R100 is tax deductable.
