I, the undersigned, hereby acknowledge and do grant the Organ Donor Foundation of South Africa (ODF) permission to use my likeness and/or statements through photographs, video images, or voice for the purpose of illustration, advertising, publication, or incorporation in film, motion pictures, internet, radio, or television productions in conjunction with efforts to educate the public about organ and tissue donation. No payment or consideration was made to me here under for this purpose. I also release and hold harmless the ODF, it’s agencies and clients, from any and all claims or actions arising out of the use or in connection with, services rendered by the undersigned and the use of the undersigned and the undersigned’s likeness for the aforementioned purposes.
